What Is Community-Based Habilitation?

Community-Based Habilitation supports individuals in acquiring, maintaining, and enhancing the skills they need to live more independently and integrate into their local community. Services are delivered outside of the home, in real-world settings—such as parks, libraries, job sites, stores, and community centers. Our highly trained direct support professionals (DSPs) provide one-on-one or small group guidance tailored to each individual’s goals, abilities, and interests.

What We Focus On

Whether someone wants to learn how to use public transportation, shop for groceries, or volunteer at a local organization, we create a flexible, customized plan that fosters growth and achievement.

Key areas of development include:

Daily Living Skills: Budgeting, shopping, using a schedule, preparing simple meals
Social & Communication Skills: Interacting with peers, self-advocacy, personal boundaries
Mobility & Navigation: Navigating community spaces, safety awareness, public transportation
Recreational Activities: Attending local events, joining community groups, exploring hobbies

How It Works

Each participant works with Colombe Home Care staff to create a person-centered plan that reflects their preferences, abilities, and long-term goals. Our services are flexible and can be scheduled around the individual’s daily life, school, or employment.

competence and confidence along the way.
