What Is the Family Supports Waiver?

The Family Supports Waiver is a Medicaid-funded program that helps individuals with intellectual and developmental disabilities receive services in their homes and communities. The goal of FSW is to promote independence, support caregivers, and
help individuals lead fulfilling lives. As an approved provider in Indiana, Colombe Home Care delivers a range of services
under the FSW designed to build life skills, enhance community access, and support daily living.

Services Offered Through the FSW

Community-Based Habilitation (CHIO): Skill-building and social development
through community-based activities
Respite Care: Short-term relief for families and caregivers, ensuring loved ones are cared for while you rest
Personal Assistance Services (PAS): Help with daily living activities such as bathing, grooming, and meal prep
Transportation: Safe, reliable rides to appointments, jobs, or community outings
Care Coordination Support: Assistance in navigating services, tracking
progress, and updating care plans

Who Qualifies for the Family Supports Waiver?

To qualify for the FSW, individuals must:

Be diagnosed with an intellectual or developmental disability
Be eligible for Medicaid
Reside in Indiana
Meet criteria for level-of-care as determined by the Bureau of Developmental
Disabilities Services (BDDS)
